Long Term Care Stella Maris has been the Baltimore-area’s recognized leader in long-term care since 1953. Upon arrival, each resident is assessed for their potential under a restorative model of nursing, with the goal being that long-term care residents should be encouraged to reach their highest level of functioning and their highest quality of life. We understand the unique differences among those we serve and we formulate a care plan that responds to individual needs. Each of our 10 distinct nursing units is staffed and programmed to best serve the residents on that unit. The interdisciplinary teams on the units include a physician, nurse practitioners, nurses, therapists, social worker, dietitian, activity specialist and chaplain. Families are asked to participate in care conferences with members of the interdisciplinary team.
